    But why doesn't he change it?

    Is there any communication explaining how to effectively categorize expenses for tests, consultations, hospitalization and expensive treatments?
    How to organize partial reimbursements from social security and mutual insurance companies?

    I agree with you that new categories need to be added.
    However, I find it less relevant to lump together personal development, gym memberships, naturopathy for example, and hospitalization costs, medical examinations, treatments for serious illnesses etc ...
